You choose the benefits and deductible, receive bills for most outpatient treatment and submit them to your health provider for reimbursement. Your policy sets the scope of cover and its end date.

Tariffs such as Hi.Germany are designed for qualifying newcomers from outside the EU, EEA and Switzerland with a befristeter Aufenthaltstitel. Permanent residence is a different situation: once you receive an unlimited residence title, you need to move to regular cover rather than keep the temporary tariff.

five years
Hi.Germany and ottonova’s special expat tariff are designed for a stay of up to five years. This is a maximum, not a promise that everyone receives five fresh years. Ask how any previous temporary cover counts towards your remaining term.
Eligibility

Many self-employed people can choose PKV, while employees generally need to meet the private-insurance eligibility rules. The general employee income threshold is €77,400 in 2026. Freelancer status alone is not enough: prior insurance and special rules, including KSK, can matter.

The comfort option includes a single hospital room and private doctor treatment within the tariff rules. Optional Hi.Dental L provides dental benefits up to €1,500 a year. It uses the same P500 medical deductible structure, with inpatient medical treatment outside that deductible.
Compare the complete cost: These are published tariff components for one 30-year-old, before any individual risk surcharge. Mandatory private care insurance (Pflegepflichtversicherung), sickness daily allowance and other additions are extra. Self-employed people generally pay the full premium; eligible employees may receive an employer contribution.

Benefits vary
by tariff
Lower-cost options can have narrower benefits than a strong lifelong PKV policy. For example, Hallesche’s Hi.Medical S excludes outpatient psychotherapy and glasses; dental cover is a separate option. Comfort tariffs can offer more. Check maternity, ongoing conditions, therapies, aids and dental limits before deciding.
You still pay
some costs
A deductible is a real expense, not just a way to lower the premium. Bills above reimbursement limits or for excluded services can also remain yours. Keep room in your budget for those costs and invoice payments. Each family member normally needs their own health cover and premium.
Your cover has
an end point
The temporary tariff cannot be kept indefinitely. Five years, permanent residence or another eligibility change can trigger the need for regular cover. Without age-related reserves in the temporary health tariff, you have not built that provision for later years. A lifelong policy can therefore mean a higher premium.

Do not wait until the final month. If you receive permanent residence or decide to stay longer, contact your health provider and specialist promptly. Ask for the exact end date of the temporary tariff, the available follow-on policies and the steps needed to keep your cover continuous.
Check the route
to regular cover
Hallesche advertises an option to continue in its open comprehensive tariffs, subject to the contract’s conditions. ottonova states that expat customers can move to its regular tariffs without another health assessment. Check the deadlines, exclusions, future premium and dental arrangements in your own policy.
Moving into GKV is not automatically available when a temporary tariff ends. Your work, prior cover and legal status still determine eligibility. Choose the continuation route before ending your existing insurance.

Incoming health insurance FAQs
Is a temporary permit essential?
Yes, for special tariffs such as Hi.Germany and ottonova’s expat cover. A visa and a residence permit are not always the same document. Ask which residence title the provider accepts. An unlimited residence title requires a move to regular cover, even if the five-year maximum has not yet been reached.
Can every freelancer join?
No. You must qualify for private insurance and meet the tariff’s residence and application conditions. Your nationality, existing cover and health assessment matter. Some freelancers, such as artists and publicists covered by KSK, have special statutory-insurance rules.
Is cheap cover always limited?
A low premium does not by itself tell you the quality. Removing age-related reserves can reduce the cost while keeping substantial benefits. Basic tariffs can also save money by limiting services. Compare the actual policy wording, especially dental, psychotherapy, aids and hospital benefits.
Is care insurance included?
Do not assume it is included in the headline health premium. Comprehensive private insurance normally also requires separate mandatory private care cover. Ask for one complete monthly figure including care insurance and any sickness daily allowance you need.
Can my family be covered?
Family members may be eligible, subject to their residence status and the tariff conditions. Health insurance is generally arranged and priced for each person. Compare the household total and check pregnancy, children’s benefits and any waiting periods or limits in the offer.
What happens when I leave?
Notify the health provider and confirm the cancellation date, required evidence and notice period. Make sure your next country’s cover starts when needed. If you stay in Germany instead, arrange a continuous move to eligible regular insurance before temporary cover ends.
